Physical Therapist – Per Diem, NMC IP

Evaluates patients and establishes an individualized plan of care. Determines patients’ functional goals and establish time-oriented, measurable goals and documents accurately in EMR. Recommend appropriate discharge Destination. Engages family and caregiver in education regarding the plan of care and discharge planning.
Knowledge of the treatment plans for specific diagnosis and conditions ie.Cardiac, pulmonary, orthopedic, Neurological, and Cognitive conditions along with accidents and falls. Recommend Medical Equipment for individuals, collaborate and communicate with health care professionals, patients, and families while advocating for the patient’s rehabilitation.
Minimum of a Bachelors degree from an accredited Physical therapy Program is required.
Current NJ Physical Therapy License is required
American Heart Association Basic Life Support is preferred
Acute Care Hospital experience in Physical Therapy is preferred.
