Candidate will be a new graduate RN to receive training and mentorship to be able to make home visits to predominantly adult and geriatric population in Union OR Somerset county, responsible for assessment and establishing plan of care for client in collaboration with patient, family and MD. Home care experience preferred. Wound care and IV skills helpful. Computer skills essential.
Principal Accountabilities:
1. Utilizes the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served.
2. Collaborates with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
3. Demonstrates and embraces the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of the job.
4. Maintains accurate, complete health care records and reports.
5. Monitors, reports, and records symptoms or changes in patient conditions.
6. Performs other related duties as assigned.
Required:
1. Bachelor’s degree in Nursing preferred
2. Graduate with diploma, associate or BN in Nursing
3. Current state licensure as a Registered Nurse or pending and BLS certification.
