Atlantic Health System is seeking a per diem Social Worker (MSW) for Case Management at Hackettstown Medical Center to work 8AM-4:30PM including one weekend every four weeks.
Essential Functions/Responsibilities:
- Discharge planning from an acute care hospital.
- Supportive and therapeutic counseling
Minimum Education:
- High School Diploma or GED required.
- MSW and LSW
Minimum Experience:
- 1 year medical setting (acute care hospital) or 1 year externship would be consider preferred.
