Elder Life Specialist Job Description:
- Identify appropriate candidates based on clinical documentation and clinical assessment of needs for Dementia, stroke depression, anxiety delirium management orders triggers hearing and visual impairments.
- Assess mental status of patients visited using Cam tool during visits.
- Clinically assess need for counseling and education to patient (if applicable) and family at bedside
- Collaborate with other staff in the development and implementation of initialized therapeutic interventions incorporating the delirium prevention protocol.
- Collaborate with other staff in providing appropriate behavior management techniques for patients with dementia
- Educate families on delirium and counsel on techniques to manage behaviors associated with delirium
- Assist with counsel to families preparing to take patient home with signs of fluctuating delirium
- Document clinical impression and interventions used with patients seen in order to facilitate continuity of care among Elder Life specialists
- Assist in training and supporting volunteers as volunteer program develops
- Maintain activity supplies on the units
Education Requirement:
- Bachelor’s degree in social work psychology or sociology
Experience:
- No licensure requirements
- Experience or keen interest working with older adults.
