Atlantic Health System is seeking a full-time Clinical Supervisor, Registerd Nurse to work at Family Practice-Overlook in Summit NJ.
Responsibilities
Essential Responsibilities
- Utilizes nursing assessment skills effectively. Coordinates and completes patient assessment and re[1]assessment to positively impact on quality outcomes. Monitors effects of medications and reports adverse effects in a timely manner. Assess barriers to learning and functional status of patient
- Consistently demonstrates competent clinical skills. Performs direct patient care activities in support of patient care delivery and team concept. Demonstrates knowledge and skill of emergency equipment and procedures. Demonstrates knowledge of medication administration according to policy. Analyzes situations critically and makes effective clinical decisions which reflect an integration of nursing knowledge and available resources (actual and potential problems). Coordinates, evaluates and completes patient/family teaching. Identifies and reports all risk management issues in a timely manner
- Demonstrates professional practice responsibilities. Assumes responsibility for coordination and completion of all patient treatments and diagnostic process. Demonstrates ability to multi-task, prioritize care and problem solve to meet the changing needs of: (1) individual patient needs, (2) unit activities (3) family/significant other needs. Demonstrates knowledge of policies and procedures. Demonstrates leadership ability. Functions as a resource/mentor and provides support to staff members to resolve patient care and staff concerns/issues. Identifies knowledge/skill gaps and takes an active role in obtaining appropriate education and/or training. Regularly attends meetings and keeps current with changes in unit function/activities. Able to work and cooperate as a team player.
- Work closely with Manager and Medical Director to Implement process improvements — including improving existing processes and tools, training colleagues, documenting and updating knowledge management collateral, and more
- Have regular, structured meetings with staff to discuss treatments and professional issues.
- Train, evaluate, discipline, and motivate staff, along with the Practice Manager
- Conduct performance reviews.
- Ensuring all clinical team members comply with licensure and registration requirements.
- Ensuring compliance, accuracy, and quality of clinical documentation.
- Enhancing operations by ensuring sufficient staffing and adequate access to services.
- Developing programs and protocols, ensuring compliance with procedures and policies.
- Assist with facilitating the multidisciplinary care of patients.
- Work closely with the ISS team and EPIC team to implement within the EMR a navigation module to help operationalize the clinical algorithms and pathways for an eventual spine navigation program.
- Navigate as a point of contact for patients and the team to the appropriate provider and care team to address patient related health care questions, assistance with appointment scheduling, access to medical records, coordination across primary and specialty care providers, benefits and billing questions, technology troubleshooting, and more.
- Serves as a liaison to physicians for any questions and concerns and to help find programmatic gaps in the pathways and helps close identified gaps.
- Identifies, develops and monitors program quality indicators relating to Patient Care Medical Home (PCMH), ACO, and infection prevention, coordination of care, compliance with plan of care and patient satisfaction.
- Monitor healthcare gaps and patient care, such as transitional care management calls, ED follow ups, high risk patients, and best practice alerts.
- Maintain laboratory and point of care testing certifications up to date.
Qualifications
EDUCATION:
- Bachelor’s degree preferred
- BSN preferred
EXPERIENCE:
- Minimum of five (5) years clinical experience in care.
- Preferably experience in a concierge or high-end clinical setting.
- Managerial/leadership experience preferred.
KNOWLEDGE:
Knowledgeable in Windows based computer applications, including Outlook, Word, and Excel.
- Demonstrated ability to work both independently and as part of a team.
- Must be able to easily adjust to changing work environment.
- Excellent organizational, analytical, interpersonal, oral and written communication skills.
- Ability to manage multiple tasks and priorities with constant deadlines.
- Maintain patient confidentiality at all times.
- Attentiveness to detail and ability to work with minimal supervision.
- Develops, establishes, and maintains productive relationships with Spine Center physicians.
